[comp.lsi] Need help addressing Futurenet mouse card

First, let me apologize for the serious cross-posting, but this question is
such that finding anyone that can help requires (IMHO) a broad distribution.

I have a mouse I/F card that was made by Futurenet and I need to know the
functions of the various jumpers (W1-W9) on this card so I can re-address
and use it. Unfortunately, one of my co-workers discarded all of the docs
when he pulled the Opus card and the Futurenet S/W off of his IBM PC/AT.

This card is a small, 8-bit (PC-size) card which has "FUTURENET" and
"M-1 BD  REV 2" silk-screened on the component side of the board. It has a
DB-25F connector for the Centronics printer I/F and a 6-pin modular (RJ-45?)
phone jack which is the connection for the Futurenet mouse. This mouse was
private-label manufactured by Logitech and is Model No. R7-3F-FT.

The only LSI-type Si on the board is a National INS8250AN. Everything else,
except for the 1.8432 MHz crystal oscillator is SSI, MSI, or discrete. I
could calculate the addressing myself if it weren't for the PAL which is 
doing the bulk of the decode logic.

In an effort to not unduly disturb groups which I don't normally read, I have
re-directed follow-ups to myself; therefore e-mail answers are preferred. If
anyone else is interested in the answers I receive, I'll be happy to e-mail
them to you. In the unlikely event that there is a huge clamoring for the
results, I will post a summary.
