I am ninety-something percent sure that this term originated when Christian missionaries set out to 'save' the natives of various underdeveloped countries (Africa was one of the first, I think). The Christians were shocked by the rear-entry position favoured by the natives -- this was why these heathens were still heathens!! The copulated in the same manner as animals!! So the missionaries, as with so many other fun things, told the natives that this was WRONG -- and introduced them to the less natural (in my view) position with the man on top and woman on bottom that came to be known (gradually, as with all language changes and adoptions of phrases) as the Missionary Position.
