[general] thesis style in "ms"

There is some confusion over the production of a thesis using thesis style
in ms.  This is at least partially due to omissions in the first draft of
the documentation.  Specifically:

    .ST TH P or .ST TH M should be used only in the file containing the title
page and pages to be numbered with roman numerals.  All other text should
be in one or more other files, each with .ST TH (without P or M) at the 
beginning.  This will provide the correct page margins, but will not cause
roman numeral page numbering.

I recommend this method over the use of .FP to end roman numeral numbering.
