[general] new substyle for letters in `ms'

The current recommended method of producing a letter with `ms' is to start
with:

	.ST LET *substyle*

where *substyle* is one of LEFT, RIGHT, or BLOCK.  There is now a third
substyle:  INDENT.  Whereas LEFT and BLOCK align the (optional) author's
address and date on the left margin, and RIGHT aligns them on the right,
INDENT indents both by (a default) 3 inches.  If you desire a different
indent, use:

	.SE LI 3.5i

(for example) to set the letter indent (LI) to 3 1/2 inches.
