[general] 12-pitch for the laser printer is now the default

As predicted, we have switched to 12-pitch as the default on the laser
printer, effective immediately.

When using `ms', italics will result from all requests such as `\fI' or
`.I'.  To obtain underlining, the `.UL' request should be used.  Alter-
natively, all italics can be overridden with the `-f 12U' option (font
12-pitch, underline).  The normal command will be:

	ms -p hpr file . . .
*or*	ms -p hpr -f 12U file . . .		

and for 10-pitch:

	ms -p hpr -f 10 file . . .

Boldface and italics will be available in 10-pitch only by special
arrangement.

The default for `format' will remain underlining.  The command is:

	format -hp file . . .

and for 10-pitch:

	format -hp -10 file . . .
