  As you noticed, .38 spal. are more and more rare. This is because the
.357 Magums do the same job, and can use almost any type of ammunition that
you want from the .38 spal Wadcutter to the strongest .357, without any
security problem. I guess that most of the american police services which
were previously using revolvers, were, in fact, using .357. In addition,
very few people would buy a .38 , for the same reason.

  I am not sure that it really fits your needs, but my revolver is a S&W
686. It has an excellent trigger, is very accurate and reliable. Finally,
its intermediate frame (L) betrween the light K and the heavy N frame makes
it quite light: I don't have any problem to fire it with a single hand. Mine 
is a 6 inches (so, you can guess that I am not using it for self defense),
but S&W has 4 and 2.5 inches barrels for this revolver. So, I hope that this
can help you.

I was half done writing a response yesterday when I reread the request and
saw that adjustable sights were a concern.  Smith and Wesson make plenty of
.38 specials but not exactly what you are looking for.  They have just about
everything else.  Now, last night I went to the range and saw a Rossi .38
special, stainless steel, adjustable sights, 4" barrel, 6 shot revolver.
(It looked like stainless anyway - could it be nickel? I don't know much
about Rossi.)  Made in Brazil.  The gun is a Smith copy, similar to Taurus. 

I looked at the gun fairly closely.  The trigger felt a little on the heavy
side.  The grips don't mate nicely with the frame (as do on my Smith), but
they seemed O.K. (slight gaps).  The price was $229.  This is a NEW gun.

As other posters have mentioned, there are Smiths (and probably others) that
come real close to your "needs".  This one seemed to have everything you 
ask for. (I don't know about the +P aspect.).   I would buy a Smith or
something else, I was not impressed by this gun.  Does Colt make anything?

Well the S&W 66 which you mentioned is a K frame (only thing smaller is the J
frame snubbies).  there is also the 67 (I like mine a lot) which is a stainless
K frame 38.  If you go to 686 you get L frame which is bigger, but basically
the K frame is a medium frame.

I don't understand what the fuss is about.  A ruger sp-101, a Taurus .357 or
the Rossi .357 are all adequate firearms to use for .38spcl and are built
heavier for durability and the mag load, why even bother with the old .38spcl?
