    Refunds for cancellations will be given per the following schedule:
	- Before May 16, 1991: Fees refunded less $50.00.
	- After May 16, 1991, but before June 3, 1991: 50% of fees refunded
	- Medical document signed by physician:  Full refund
	- No refunds will be granted after June 3, 1991
	- Cancellations must be in writing and postmarked by June 3, 1991 to
          receive a refund
