I am looking for a good (the best, if possible) software method of
converting my IBM XT QWERTY keyboard to DVORAK, and perhaps vice
versa.  Any suggestions will be greatly appreciated.

I may be mistaken but I think FansiConsole has a Dvorak 
keyboard translation. Even if it doesn't you should 
look into it for your XT. I gives the impression of speed,
among other things. 

Have the No Smoking Software people done anything since? 
256 color VGA support would be nice...

You can try FANSI-CONSOLE.  It does keyboard remapping, and dozens of
other things.  It's shareware so you may be able to find it on a BBS
somewhere.
