I have an EGA card that I need to identify...

It is a half size, 8 bit card.  Accessible form the exposed end, it has
one female DB9 connector, 2 RCA connectors, and a bank of 5 DIP
switches.  The most distinguishing markings on it is a sticker across a
large 40 pin DIP IC that reads "Sigma Designs, Inc.  EGA 480".  It is a
yellow sticker, and has emblem that looks like a filled in letter D with
a greek sigma inside of it.  There is also another (for a total of 2) 40
pin DIP IC.  There is one 68 pin PLCC "CHIPS P82C431" ic in the top left
corner, a 84 pin PLCC "CHIPS P82C434A" ic in the middle of the board,
and another 84 pin PLCC "TAG 8732" no brand chip in the lower right
corner.  There are 8 "D41464C -12" 64K? ram chips in the middle/lower
left side of it.  There is a row of 74LS244 and 74LS245 chips across the
bottom close to the card edge connector.  There is a 32 pin female plug at the
center top of the board.  There a 6 pin header with pin 2 cut off at the
top right of the board labeled P1.  Looks like a game connector?  There
is a 3 pin jumper labeled P2 under P1.  There is another 3 pin jumper
labeled P3 in the lower right of the board.  There is a "27256" eprom in
the lower center of the board.  On the front, there are markings "P/N
150-318 EGA 480 Rev 1.50".  On the back there is "KALEX K668  94V-O
2487".  

Thats about all I can say about it.  Can anyone tell me who makes (made)
it and a phone number?  An explanation of the 5 dip switches would be
great.  I would like to install it in an XT clone, but I am not sure if
the BIOS supports it and how to overcome that...
