[comp.os.msdos.programmer] Looking for SVGA programming info

I am interested in programming a VGA card that uses the Tseng chip set. 
I have been through a great deal of public domain packages, but most
of them try to be general and only support up to the 800x600x16 modes.

What I would like to know, is how the 1024x768x16 mode is set up (mode
37h under Tseng.)  Is this mode an extension of EGA bit plane modes,
requiring the programming of the standard EGA ports 3CEh 3C4?  If so,
does someone have a complete list of port services?  What is the
memory scheme and how do the port 3CDh bankselect read/write bits
relate to the screen?

Please point me to the right Assembler code.  
Also has anyone read the new McGraw-Hill book on advanced VGA programming?
