My copy has pages 413--416 in it.  The article is entitled
"Textbook Publishing --- 1990 and Beyond".

Someone from TUG contacted me by e-mail and said that I was the eighth
person who had complained about missing pages.  They are requesting
people with defective copies to return them to TUG, which will then
send them to the printer.  TUG will replace defective copies with good
copies.
