[comp.text.tex] need a fancy box

I'm fairly new to TeX, and I suppose I know enough that I could
bang away at this for a couple of hours trying to figure out how
to do it, but I have a fairly immediate need ...

I'm not very good at making boxes yet.  I need one of the
following description, complete with hrules and vrules as pictured:

The contents of the name field will be filled from a database
program and so will be different for different printings, and the
ABC part of (nnABC) could be 1 2 or three letters. The little box
to the left of each name is for a checkmark, maybe a quad in
width.

The box needs to be printed flush right regardless of the length
of the names I put in it, so it can't used fixed spacing.

Notice the cutout in the upper right of the box.

If someone knows an easy solution to this probably
not-too-difficult problem and could email it to me, I would be
greatly indebted.
