I asked this question once before but got no answers. 

Does anyone have the bib-style used in Graham, Knuth, Patashnik's
"Concrete Mathematics"? In Concrete Math the bibliography is
organized so that the numbers of the pages where a certain bibliographical
entry is referenced are printed in the margin next to the entry.

Actually, what I want is something better than what is used in 
Concrete Math. For example, entry nr 173 is said to be
referenced on pages 602, 603, 604, 605, rather than on 602--605.

look in LaBrea.Stanford.edu under the DOC directory for the file GKPMAC.TEX.
This is the macro package that Don used for the book.
