Hi Linda, I'll do my best to briefly describe formmate. It is basically a 
software packages that allows a blind person to fill out forms. It can either 
print the entire form with blanks filled in, or it can print only the answers 
on a pre-printed form. You must of course get the original form into the 
computer one way or another. This can be done by scanning, having someone type 
it in or whatever. 
    The contents of the form are stored in one file and the contents of the 
answers are stored in another. This allows you a lot of flexibility in what 
you do with the data. You can for example use the name address and phone 
number of a person in filling out more than one form without having to reenter 
the data. 
    Formmate has built-in logic, for example, when you need to choose from a 
list of shipping methods, once you make a selection the program skips to the 
next question without reading all of the remaining choices. If you check a box 
that says single the program will automatically skip all questions such as 
spouses name. It seems to work very well and I think it would be of great help 
to someone who does a lot of paper work. 
    Formmate will work with screen readers other than jaws. I hope this
