  Hi there,

I am an ALS patient and I'd like to hear from ALL ALS PATIENTS
out there as I'd like to begin building an ALS data base.  I've
been toying with the idea of putting together a questionnaire to
see if any of us have things in common which just may aid in ALS
research.

        NAME
        NETMAIL ADDRESS
        HOME ADDRESS
        YEAR OF DIAGNOSIS
I should be in contact with each of you shortly.  Please indicate
whether or not you'd like to be in contact with other ALS
patients, so I can send you a list of others.
