A few weeks ago there was a discussion of the Manned Maneuvering Units to be
used on upcoming shuttle missions and the issue of MMU faliure was brought
up.  The 23-January-84 issue of AW&ST has a detailed article on the MMU's and
NASA's emergency procedures are mentioned:

The units have a fail-safe/fail-operational design and it is expected that
enough backup capability would remain to allow the astronaut to fly back to
the shuttle in the event of a system failure, however...

 "In the unlikely case of the system becoming totally disabled, stranding
the untethered astronaut far away from the orbiter, the orbiter crew would
fly the shuttle close enough to the stranded crewman so that a second EVA
crewman in the payload bay could grab him."
