In reference to CHKDSK's message about 0 bytes in 1 hidden file, if I remember correctly, CHKDSK is probably registering the volume label, in which case PCTOOLS does show it (at the top of the screen, instead of in the file listing). Try installing the system onto the disk (i.e. SYS A:), and then run a CHKDSK. It should register xxxxxx bytes in 3 hidden files, where xxxxxx depends on the version of the system that you are using. Respectively, the hidden files should be: IBMBIO.COM -- Contains the BIOS routines IBMDOS.COM -- Contains the DOS routines (volume label) IBMBIO.COM and IBMDOS.COM will appear in the PCTOOLS window.
