Mainly, the concensus among the experts seems to be that such extensions should be handled by filtering programs and not directly by printer drivers. There is a program called ivd2dvi, by Larry Denenberg, available by anonymous ftp from june.cs.washington.edu (as part of the standard Unix TeX distribution) which converts dvi-ivd files to ordinary dvi files which can then be printed by any printer driver.
