For those who are interested, here's what I've found out about my manual feed  
and page ordering questions:

The question of whether manual feed can be selected on a page-by-page basis is,  
for this version of NeXTstep, at least, no.  The printing architecture just  
doesn't handle this type of thing on a page basis -- it sets the printer up for  
the "job", and then images the pages using the window server.

Page reversing *can* be controlled on a document basis, however, even if it's  
not done the same way as documented in the Adobe manual.  Basically, you  
include a number after the %%Pages: (atend) comment at the start of the  
document.  1 means the following pages are in ascending order, and should be  
reversed; -1 means the pages are in descending order, and need not be reversed;  
0 means the pages are in a special order, and shouldn't be touched.
