There has always been a way to ensure fighting until near death.  It's
called typing the direction key pointing at the monster.  I admit this
isn't as fast as 'f'dir, but if you're that worried about dieing, what
are you typing 'f'dir for anyway?  Also, I think that monsters can hit
for extra damage (like in D&D).  The 'f' cannot really take this into account
in figuring 'near death'.  If it takes the monster's normal damage into
account, you will fight until near death.  If the monster really whacks
you, you die.  Therefore, fight that 'U' slowly (unless you're wearing
AC15 armor).  This also gives you the benefit of chickening out when
you still have enough HP to withstand a hit from another wandering monster.
