First of all, here are the answers to the teams that move:
Brooklyn -> LA Dodgers
Minneapolis -> LA Lakers
Oakland -> LA Raiders
Los Angeles -> San Diego Chargers (A LOT of people guessed Dallas here)
New York -> San Francisco Giants
New Orleans -> Utah Jazz
The "Los Angeles" Rams play in Angel Stadium, which is in
Anaheim, which is in Orange County. (Orange County may be
physically close to LA county, but culturally, they're worlds
apart)
I got an awful lot of answers of other teams that have moved
once, or twice, and/or changed their names. I will try to summarize
below (By the way, I am just reporting these. Don't blame me if
they are wrong) :
Football:
Chicago -> St. Louis Cardinals
Dallas Texans -> Denver Broncos
Baseball:
Boston -> Milwaukee -> Atlanta Braves
Philadelphia -> Kansas City -> Oakland Athletics/A's
Seattle Pilots -> Milwaukee Brewers
St. Louis Browns -> Baltimore Orioles
Washington Senators (I) -> Minnesota Twins
Washington Senators (II) -> Texas Rangers
Basketball:
Rochester -> Cincinatti Royals -> Kansas City Kings
New York -> New Jersey Nets
Philadelphia -> Golden State (Ca.) Warriors
Hockey:

And it was also mentioned by some people that, like the Rams,
the NY Giants also don't play in their home city. As a matter
of fact, not even in their home state. They play in New Jersey.

Some comments on teams that moved: The Kansas City Scouts hockey team becams the Colorado Rockies. The Atlanta Flames were in expansion team that entered the NHL in 1972, the same year as the New York Islanders. (Speaking of hockey moves, one could fill a screen with the moves of the old WHA teams, but I won't go into that.) In football, it should be noted that not only have the Los Angeles Rams moved to Anaheim, but they originally moved from Cleveland to Los Angeles. Also, I believe that the Broncos began their existence in Denver, and that the Dallas Texans were actually the last NFL team to fold. In basketball, add the Buffalo Braves, who became the San Diego Clippers, and the Indianapolis Pistons, who moved to Detroit, and the Minneapolis Lakers, who moved to LA. Lest there be too much misinformation disseminated:
The Dallas Texans of the American Football League (used to have a
picture of Texas on the sides of their helmets) took their act
north and east a couple of states to become the Kansas City Chiefs.
One team that moved that is often overlooked is the Houston
Rockets, ne'e San Diego. Not only have they changed cities,
they've changed arenas in Houston ('Dome, Hofheinz, and now the
Summit). Now the extra credit quiz:
What other current NBA team used to be called the Rockets?
(they changed their name without leaving their city)

Wayne Citrin's followup article on teams that have moved contained
a couple of errors. One, the Dallas Texans did not fold; they moved to
Kansas City and became the Chiefs. Two, the Pistons moved to Detroit
from Fort Wayne, NOT Indianapolis.
A complete list of every team that has moved in the history of the
four major American professional sports would be very long. Although
quite a few of these teams have been mentioned in this discussion, many
have not. Two of my favorites are
1) Frankford Yellowjackets
2) Portsmouth Spartans.
Ten points to the person who can correctly identify the current name
of both of these teams.
