In answer to question number one: yes the 4040 has the same BASIC
interface that the 1541 does.  It has the same DOS commands in
the disk, but the 1541's memory map is slightly different.  The
4040 has the B-P and U1 stuff.

Question 2:  When loading the MCS, it comes up with the logo and
appears to load just fine, then it bumps the head on the disk,
the screen goes blue and the keyboard locks up.

As to CIOUT and ACPTR, these do not exist per se, but let me page
through my references on the PET to see what is equivalent.
The following are the same:  OPEN at $FFC0, CLOSE at $FFC3,
CHKIN at $FFC6, CHKOUT at $FFC9, CLRCHN at $FFCC, CHRIN at $FFCF,
CHROOUT at $FFD2, plus LOAD, SAVE, STOP, GETIN, CLALL, UDTIM.
The analogy to CIOUT appears to be at $F0EE which is described as
send one character on the IEEE-488 bus.  The analogy to ACPTR
appears to be at $F18C which is described as get one character
from the IEEE-488.  I'm not exactly sure why you need CIOUT 
and ACPTR is you have CHROUT and CHRIN.
