SAMPLE COOPERATOR INFORMATION SHEET. INSTRUCTIONS TO PRINCIPAL INVESTIGATOR: A signed Cooperator Information Sheet must be given to the cooperator for his/her records. COLORADO STATE UNIVERSITY Cooperator Information Sheet Project Title: Colonization rates by mice Principal Investigator: Ann Eileen Miller Baker Contact Person and phone number for questions/problems: Ann Eileen Miller Baker 303 482 1719 (evenings) AEBAKER@CSUGREEN Biology Dept, CSU, Fort Collins CO 80523 USA Objectives/Purposes of Research: Determine the date, species, sex, and age of mice colonizing homes. Develop more integrated approaches to pest management and more realistic estimates of the rate of evolution. Procedures/Methods to be Used: Over your year as a cooperator, when you see a mouse or its feces, set 10 baited snaptraps within 1 inch of objects or walls. As long as you wash thoroughly your hands and all objects that the mouse has touched, there is little health risk. Put in the freezer a plastic sack containing the dead mouse and its date of capture. Renew bait (peanut butter, cheese, bacon) so that the traps have an attractive odor. Check traps daily until 14 days after the last mouse was caught, then then send mice to Ann Baker. Before shipping mice, open body cavity and preserve in rubbing alcohol for about 1 wk - "drip dry" the carcasses, send in a "seal a meal" bag. All data are confidential. Risk: Failure to wash your hands and objects that mice have touched may put you at a risk for a human disease carried by mice. Assurance of Confidentiality: All cooperators will be anonymous. I agree that the cooperator has the right to terminate participation in this research project at any time. Ann Eileen Miller Baker Feb89 Investigator Date
