It's most likely that the problem WAS heat.  The USR Password does run
a bit warm.  However, there's another thing users should remember:
most CRTs have a rather strong magnetic field from the deflection
coils around the display tube.  Most modems have one or more
transformers, used to isolate them from the phone line and/or for
"Hybrid" balance.  If you mount the modem too close to anything that
has a strong magnetic field, you're asking for trouble.  The result
will be errors and/or decreased demodulator sensitivity in the modem.
