Well, the consensus answer to my query on the cause of unexpected error 8454 is that it's due to an internally corrupted stack, and the best way to recover from it is to use a stack called RecoverStack. A stack from sumex called Hypermerge may also be useful in recovering incorrupt cards. As to avoiding the problem in the first place, the best way is to keep regular backups.
