The original source from AT&T does not work because AT&T assumes statically allocated FILE structures, which is not quite true on this version of Ultrix. Otherwise, for an old ksh you can grab a local copy of /usr/src/lib/libc/stdio/data.c and up the number of static iob's from 3 to 20.
