[comp.unix.ultrix] Does Ultrix 4.0 come with a customer disk drive formatter?

Our VAXstations' diagnostic tape had a RD disk formatter on it.
Our DECstation 5400 diagnostic tape doesn't have anything for our RA90/RA82's.
My RA82 needs help; I guess I have to call DEC.
