What you need is a brand new book called _Bit-Mapped Graphics_
by Steve Rimmer.  Publisher is Windcrest.  Price is $26.95.
Cody's and Stanford Book Store should have it (I got mine at
Cody's).

I've just cracked the cover so I can't review it yet;  I'll post
when I know more.  A brief thumbing-through reveals a lot of C
source and a sprinkling of 8086 assembler.  Looks like a lot of
meat.
