I'm trying to find a document that specifies the layout of a CDROM, so I can generate data in this format on a 700MB disk in "CDROM ready" format. This was once referred to as the "high sierra" format. It is not a structure and index format definition, but a layout of data blocks (physical) & headers. Please E-Mail & I'll post the results.
