       This survey is intended for someone whose performance is
       reviewed by supervision.  This research is for an
       organizational communication class for which I must write a
       paper.  Please take a few minutes to reply to the following
       multiple choice questions, keeping in mind a recent
       performance review and your behavior following that review.

       1.  How often are you reviewed?

        [2]  twice a year
        [3]  quarterly

       2.  Do you think your performance evaluation evaluates you
           or your performance?
