I ran into an interesting (to me anyway) problem with our 11/750 system recently. I noticed that we had been getting a message during boot that our user disk had been improperly dismounted and that a REBUILD was in progress. This happened both when we had a crash, and when we used SHUTDOWN to bring the machine down. Along with the REBUILD, we received a message indicating that we had a duplicate quota file entry. Well, the message manual had little useful information on this particular problem. ANALYZE/DISK/REPAIR did not fix the problem. It did however point at the problem, as it complained that it could not write DISKQUOTA information on the disk for UIC [0,0], as there was already an entry for that UIC. Therefore, I ran DISKQUOTA, removed the record for UIC [0,0], dismounted the disk, mounted the disk (getting a REBUILD, but no disk quota error), dismounted the disk, mounted the disk, and VOILLA' (sp?) I had a normal disk mount. You figure it out. This seems to have fixed my problem, and will probably fix yours if you are having this problem. It would be of interest to know what mechanism got messed up.
