We have some Cipher 990 tape drives interfaced through Emulex TC13 controllers
emulating a TS11. I have seen the same problem a few times when we were running
VMS V4.5 or V4.6 on a Vax 8300. I did not further investigate the problem,
since I saw the problem only a few times and only for software distribution
kits from Digital.

Does this problem always occur or only when your default directory is set to a
search list (e.g. SYS$SYSROOT:[SYSMGR] on a VAXcluster). In the latter case the
problem does not only occur for tapes, but also for disk directories. I do not
completely understand the problem. But try the following commands when you have
a cluster:

$       SET DEFAULT SYS$SYSROOT:[000000]
$       DIRECTORY SYS$SYSDEVICE

You will also see each file listed twice.
