Looks like you crashed the phone manager.  I've seen this when you
confuse the window manager and the "[w]" icon dies as well.  Reseting
the machine is a little drastic.  You can:

	$ su 
	Password:
	# /etc/ph

	-and for the window manager-

	# /etc/wmgr

This will respawn the animal and get it running.
