[comp.windows.x] printing out the Xlib documentation

How do I print out the Xlib documentation in the /doc/Xlib directory?
I want to end up with a document that has an index, glossary, etc.
with the correct page numbers on each page. (i.e. I want the page
numbers of the index to match the pages in the chapters.)  I could just
cat all of the files together in the /doc/Xlib directory and print them,
but where do I get the index from?
