(Looks like I missed a reference). The server extensions documentation was folded into the ddx interface document. This centralized the DIX interfaces in one place and makes it more likely that they'll remain consistent (at least internally, and maybe even with the sources). Most of the old (pre-R4) interfaces were either replaced or substantially modified to make updating the old document useless; the replacements were frequently of equal utility in writing DDX routines anyway.
