I talked with Lou Katz at the USENIX Conference and the subject got around to the future of conference tutorials. He hoped that there would be interest in more tutorials and suggested one could be given on writing device drivers. If such a tutorial was developed, I should think its "lecture notes" could form the beginnings of the document you desire. I'm sure Lou would welcome opinions and suggestions via ...!ucbvax!g:usenix. For a more immediate reference, unc!smb suggests the V6 UNIX Commentaries available from AT&T or your local black market. The description there of the drivers, while V6 oriented, is quite good.
