There is a bit in the local state word of the 'new' tty driver on 4.1 bsd called 'LMDMBUF' which allows the 'carrier detect' line (rs232c pin 8) to be used to stop/start output from a dh/dm. I have modified the tty related sources several times on std version 7 and sys III systems to do the rts/cts handshaking ... it isn't very complicated.
