[comp.sys.sun] problems formatting Maxtor XT-8760E

I'm trying to format a Maxtor XT-8760E connected to a Sun Emulex MD21
controller (using the original Sun entry in /etc/format.dat for this
drive/controller combination).  I'm using format under SunOS 4.0.1 on a
Sun 3/60. The disk formats fine (and seems to work) if I enter up to (and
including) 130 defects. However, the defect list that comes with the drive
has 234 defects.  I can successfully enter all of these defects using
format (format could not extract them from the disk); however, when I try
to format the disk I get the following error:

    Block 3 (0/0/3), Fatal non-media error (illegal request)
    failed

Any combination of 131 or more defects seems to fail (and any combination
of 130 or less seems to succeed.)  Any suggestions on what to try next
would be appreciated.  Thanks,
