I don't know if this has been discussed here before, but I spent some of last night fighting this one. If you try to use the 4.0.1 "config" to config your new 4.0.3 kernel, you will wind up with all kinds of strange results. In my case it quietly made it through a "make depend" and the kernel built fine, and then died upon booting after trying to bring up zs0 at priority 6 and panicing with "wrong zs priority". Who knows what else could happen. Conclusion is: use the config that came with the given OS version...
