1988
December 1	A technician readying a Boeing rocket for launch aboard the
space shuttle slips on the hem of his lab coat and bangs into the rocket's
first stage, leaving a four-inch crack in the exhaust nozzle.  The entire first
stage had to be replaced at a cost of $6 million.  Press coverage of the mishap
omitted the tecnician's name, sparing him public humiliation.
