[comp.databases] Needed Info. about a Text Book on ORACLE 5.1

We are trying to organize a graduate course devoted to applications 
development in ORACLE (on UNIX platform), as part of our graduate program
in Management Information Systems, next Spring.  As such, I seek advice on
a good book that will serve as the Text for that class.

The book should:
    i) emphasize on SQL*Forms v. 2.0;
   ii) be a paperback;
  iii) have several example applications.

I would appreciate if any knowledgeable netter could e-mail me the required
information.
