We just got our 4.3 manuals from Usenix c/o Howard Press. We ordered 5 programmers manuals. One of them has two extra pages between the copyright page and the table of contents: first an upside-down page iii/vi, and then a rightside-up page iii/iv. The rest of the book seems to be intact, but I havn't actually counted pages or anything.
