[comp.dcom.lans] Forms for datacenter management

I am interested in what forms people use in data center management.
For instance, what do you use to record trouble calls, line statistics
(BERT results and the like), and so on?  I know most people store the 
stuff on-line, but I'm interested in the forms you use to write the data on
*as you take it* or to keep it on if you do not store it on line.
