[comp.periphs] Need help formatting with OMTI 5520 controller

I would like to format a 10 meg Miniscribe hard drive using an OMTI 5520
controller.  Does anyone know how to perform this LOW LEVEL format using the
OMTI 5520?

I have the information regarding a omti 5520A controller, I hope this is
the same.
You must set the jumpers on the omti board to W1,W2,W3 and W4 to 0 for a
306 cyl,4 heads or W1,W2 and W4 to 0 and W3 to 1 for a drive with 612 cyl
and 2 heads.
Then start DEBUG and type g=c800:6 .
You don't have to set the jumpers, you can give the parameters after
you start debug.
