I had that problem for a while, until I remembered that '+' has
special meaning to regular expressions!  Do this:
...
(setq auto-mode-alist (cons '("\\.h\\+\\+$" . c++-mode) auto-mode-alist))
(setq auto-mode-alist (cons '("\\.c\\+\\+$" . c++-mode) auto-mode-alist))
...
