Here are the suggestions and pointers I received in response to 
my request for information about KA.  I am currently evaluating
Brule and Blount, and McGraw and Harbison-Briggs.

Allow me to suggest "Knowledge Acquisition," by Brule' & Blount, 
McGraw-Hill, 1989. This non-biased observer thinks it's a great
book! :-)

A new book, just published I believe, is "A Practical Guide to Knowledge 
Acquisition", by C. Scott, J. Clayton, and E. Gibson.  I haven't read it
yet, but I can strongly recommend the authors (having worked with them
a few years ago).
