[comp.text] \topinsert and \topskip

Help!

Could someone explain to me why \topinsert doesn't use \topskip?

My problem is that I have a big headline that requires me to set
\topinsert to 10mm. When I only have plain text on a page everything
looks all right. But when I have a \topinsert on the page the
topinsertet material begins "in" the headline, ie the \topskip is not
used.

Preferably I would like to have a macro that I could use in the middle
of a paragraf, just like \fig {this is an example} {\vbox{}}, that
would leave "figure <somenumber>" in the text and topinsert some
material - \vbox{} and {this is an example}. One problem is that the
TeX book says that topinsert should only be used between paragrafs...

No doubt the answer is in the TeX book, but where?
