Is there a public domain program that reads a LaTeX ".idx" file
and creates a useable, alphabetized index like this:
\begin{theindex}
\item ...
\item ...
\subitem ...
...
\end{theindex}
??
If so, does it run on UNIX?

Try "makeindex" from the Clarkson LaTeX archive server.
