Kent,
Although you may mean well by your survey, some of the questions that you
ask are rather personal (salary? benefits?), especially for a mass-
distributed survey like this. The feeling is kind of like asking
someone you just met at a party how much money do they earn or what
disturbs them the most about their sex life.
I guess my major concern is that you don't mention what the data
will be used for. I think I would be more amenable to offering
information if I knew what the survey was about and how the info
would be used.
