[comp.graphics] Siggraph '84 Course Notes Request

Does somebody have a copy of Volume 12 of the Siggraph 1984 course
notes?  (Sorry, I don't know the course title.)  I'd like to get a
Xerox of the following reference, mentioned in Eric Haines' fine
section in the 1987 Ray-Tracing notes:

    Blinn, J.F., "The Algebraic Properties of Homogeneous Second
    Order Surfaces," SIGGRAPH Course Notes, Vol. 12, July 1984.

I don't want the whole set of notes, just that section of Blinn's.
I'd be willing to spring for a dozen dollars plus copying costs.
